Two-Sided PDF Statute Forms

I need a cleanly designed, two-sided fillable PDF that functions like an official statute form. Every area the end user must complete should appear as a text field, while several header and reference sections will arrive from me already populated and must stay locked in place. The finished file has to open smoothly in Acrobat (and common browser viewers), allow tab-to-next-field navigation, and print correctly front-to-back on standard US letter without shifting margins. Once I give you the pre-filled wording and a rough layout sketch, you will arrange the elements so the first page flows naturally onto the reverse while preserving ample white space for handwritten signatures if needed. Accessibility tags and basic form validation (e.g., required fields, simple character limits) are appreciated but not mandatory; clear, consistent styling is. Deliverables: • One interactive, press-ready PDF (double-sided) • A flattened proof for quick review • Source file if you build it in InDesign or similar, so I can update statutes later I’ll approve the project once every text field is live, the locked language can’t be edited, and both sides align perfectly when printed.
