I want to turn a single state family-law statute into two parallel, fully fillable forms—one designed for the self-represented litigant and one for the judge who will rule on the matter. For the litigant version, every statutory element must appear in logical order with a corresponding field. The form should capture personal information, the relevant case details, and any supporting documents, walking the user point-by-point through the language of the statute so nothing is missed. The judge’s companion form should mirror those same sections but swap input fields for quick-check boxes and open text areas that let the bench record findings, notes, and the ultimate decision directly against each provision. Key goals • Both forms read exactly like the statute, preserving headings, numbering, and wording. • Fields are clearly labeled, self-explanatory, and impossible to skip when the statute calls a point “mandatory.” • Layout remains clean whether you build in Word, Acrobat, or another standards-compliant tool; I simply want files I can circulate immediately. • When I open each document I can type, tab through, save, and print without losing formatting.
