Cardiology Patient Histories Creation -- 2

I need a skilled medical writer to draft clear, accurate patient-history records for cardiology cases. Each history must read like it belongs in an electronic health record: concise narrative, logical flow, and fully compliant with common clinical documentation standards. These histories will focus on a single specialty—cardiology—and must capture the patient’s current and prior medications and treatments in detail. Surgical background, family cardiac risk, and other elements are not required this time, but if our collaboration goes well I may ask for them later, so structuring the files so they can be expanded is helpful. You will receive anonymised source notes (consult letters, lab summaries, imaging reports); your job is to transform them into a polished, physician-ready history section.
