This is a focused three-day sprint to transfer patient information from a mix of scanned images, PDFs, Word files and faxed pages into my master record set. As you enter each item you will: • Verify that every required field is present, flagging and annotating any gaps. • Spot and remove duplicate or contradictory entries. • Prepare and file the associated PDG forms once the data is clean, making sure all identifiers match the source documents. The finished deliverables are: 1. A fully populated digital record (Excel or Google Sheets is fine) with zero duplication. 2. A folder of correctly filed PDG documents. 3. A brief log noting any files that were incomplete or unclear. Accuracy and speed are both critical because of the tight timeline, and I expect the work to be done entirely by you—please avoid AI-generated content in both your application and the actual data work. If your background includes hands-on medical record entry, experience reconciling scanned/faxed charts, and a track record of meeting tight deadlines, this project should be straightforward.
