I need a clean, responsive contact form that slots straight into my existing website. The form has to collect the visitor’s Name, Email and Phone Number, and it also needs fields for Address, Subject line and Company name. Once a user hits “Send”, the information should be routed to my chosen email inbox and stored securely in a simple database or CSV for easy export. Error handling, basic spam protection (reCAPTCHA or similar) and success / failure messages must be in place so the user experience stays smooth. Please deliver: • Fully-styled HTML/CSS markup • Any PHP, JavaScript or backend code required to process and validate the submission • A brief read-me explaining how to deploy the form on my server and change email or database credentials I’ll test by submitting sample data; as long as every field reaches me accurately and the page remains responsive on mobile and desktop, the job is done.
