The task is pure data collection and input, with no cleaning or analysis required on your end; accuracy is the only priority. Each row in Excel should mirror one line of the original form, using the column headers I will provide. I will supply high-resolution scans (PDF) plus a small sample file illustrating the expected layout. Once you finish a section, send the sheet back so I can spot-check before you continue; this rolling review helps avoid any re-work later. Deliverable • Completed Excel file containing every record from the printed documents, ready for immediate use. Acceptance criteria • 100 % of fields transcribed. • Less than 0.5 % transcription error rate (I will audit a random sample). • Workbook returned in .xlsx format, no macros.
